Lighthouse Behavioral Wellness Centers in Durant, OK is seeking a Care Coordinator to empower individuals healing from mental illness, trauma, or addiction. You will advocate, link consumers to services after crises or hospitalization, and collaborate with the team to support recovery.

Under supervision, you provide case management, rehabilitation and wellness services across settings, complete assessments, care plans, and coordinate with providers.

Competenze

• Bachelor or Master in education, psychology, social work, sociology, criminal justice or similar behavioral health field approved by ODMHSAS.
• Current RN license in Oklahoma with behavioral health experience.
• Bachelor’s or Master’s degree in any field with 6 months of behavioral health experience.
• 60 college credit hours and 12 months of behavioral health experience.
• 36 months of direct, documented behavioral health experience.

Mansioni

• Provide case management, rehabilitative and wellness services in various settings.
• Coordinate wraparound family team meetings and include formal/informal supports.
• Enter data into the YIS system per ODMHSAS requirements.
• Provide crisis management and stabilization as assigned.
• Develop care plans and coordinate with internal and external providers to monitor progress.
