Job summary

Lighthouse Behavioral Wellness Centers is seeking a Care Coordinator to empower and support individuals recovering from mental illness, trauma or addiction. You will link clients to services after crises or hospitalization, coordinate care with a multidisciplinary team and provide community-based support to promote stabilization and recovery.

The role requires knowledge of behavioral health principles, wellness coaching and strong collaboration across inpatient and outpatient settings.

Qualifications

• Requires knowledge of social work/psychology concepts and current social welfare resources.
• Familiarity with the Eight Dimensions of Wellness and helping clients develop personal goals.
• Ability to work in integrated inpatient/outpatient teams to assess needs, plan care and monitor progress.
• Coaching approach for collaboration and engagement with diverse populations.

Responsibilities

• Provide case management, rehabilitation and wellness services in multiple settings.
• Deliver crisis management and stabilization as needed.
• Assess consumer needs, evaluate treatment options, create care plans and coordinate care with providers.
• Complete all agency documentation accurately and timely.
• Assist clients to adjust to their environment and treatment.
• Create partnerships to expand access to housing, employment and health resources.
• Attend huddles, care planning and staff meetings; meet productivity standards.
• Travel as required for community-based services and home visits.

Bachelor or Master in Education/ Psychology/ Social Work/ Sociology/ Criminal Justice

RN license in Oklahoma with behavioral health experience

Bachelor’s or Master’s in any field with 6 months behavioral health experience

60 college credit hours + 12 months behavioral health experience

High school diploma + 36 months direct behavioral health experience
