Job summary

Family Health Center is seeking a Care Coordinator to link behavioral health, substance use treatment, and primary care with community resources. You will help clients understand and follow through with care, address barriers like transportation and housing, and coordinate with internal teams and external partners to improve health outcomes.

Typical duties include scheduling, follow-up, and updating care plans while maintaining accurate EHR documentation.

Qualifications

• High School Diploma or GED required.
• Experience in behavioral health or care coordination preferred.
• Ability to work with diverse populations and communities.

Responsibilities

• Coordinate care across systems and providers to ensure seamless service delivery.
• Assist individuals with scheduling, transportation, housing, and benefits access.
• Follow up on appointments and track client engagement in care.
• Communicate with PCPs, therapists, and other providers to support treatment plans.
• Develop and update person-centered care plans and safety plans.
• Document activities accurately in the EHR and participate in team meetings.
