Job summary

Lighthouse Behavioral Wellness Centers is seeking a Care Coordinator to empower clients and link them to services after crises or hospitalization. You will work under supervision to provide case management, rehabilitation and wellness support to individuals and groups.

A core focus is on improving outcomes through individualized care planning, collaboration with internal and external providers, and by utilizing Eight Dimensions of Wellness. Community-based work and travel may be required.

Qualifications

• Knowledge of social work and psychology principles and related health resources.
• Understanding of crisis intervention and recovery support practices.
• Ability to work with diverse populations and cultural backgrounds.

Responsibilities

• Provide varied case management, rehabilitation and wellness services in multiple settings.
• Deliver crisis management and stabilization as assigned.
• Assess needs, help plan care, and coordinate with providers and community resources.
• Document services thoroughly and timely in agency records.
• Assist clients to adapt to their environment and treatment plans.
• Participate in huddles, Care Planning and staff meetings; travel as required.

Current OK nursing license with behavioral health experience

60 college credits + 12 months behavioral health experience

HS diploma + 36 months direct behavioral health experience
