Qualifications

Responsibilities

• Collaborate with outpatient clinical teams to develop Individual Service Plans.
• Develop care coordination protocols with appropriate consent for release of information.
• Coordinate care across physical health, behavioral health, social services, housing, education, and employment.
• Obtain and document appointments with other entities in clinical records.
• Develop crisis plans with clients for psychiatric or substance abuse crises.
• Coordinate care to enhance treatment engagement and continuity of care.
• Coordinate transitional care between hospitals and community facilities.
• Engage clients across the lifespan focusing on skill development and access to resources.
• Provide ongoing assessment of clients' cognitive, emotional, physical and vocational needs.
• Address activities impacting behavioral health such as sleep, exercise, media, and nutrition.
