Job summary

Pueblo Community Health Center is seeking a Behavioral Health Care Coordinator to support access to behavioral health services for the patient population in Pueblo, CO. The BH Care Coordinator manages and directs incoming phone calls and clinic BH triage, and schedules BH therapy and psychiatric treatment.

Requirements include a Bachelor’s degree with three years in community health, strong communication, and organizational skills; Spanish is desirable; a valid driver’s license is required;

Qualifications

• Bachelor’s degree with three years in community primary health care.
• Strong leadership, advocacy, and communication skills.
• Experience with scheduling and coordinating behavioral health services.

Responsibilities

• Manage BH team calls and triage, schedule BH therapy and psychiatry.
• Generate reports and coordinate patient access to services.
• Maintain documentation in the medical record and ensure confidentiality.
