Qualifications

Responsibilities

• Facilitate outreach to establish referral providers and participate in outreach events.
• Facilitate intake admissions and reassessments as scheduled.
• Maintain and organize patient records in compliance with policies.
• Coordinate treatment planning and linkage to appropriate levels of care.
• Ensure compliance with federal/state regulatory and accreditation requirements.
• Attend assigned work location and meet scheduling expectations.
• Develop formal treatment plans within required timelines.
