Job summary

St. John’s Community Health in Compton, CA is seeking a Behavioral Health Care Coordinator to ensure timely navigation and coordination across IBH services, including therapy and psychiatry.

The role partners with the BH Fusion Lab to improve access and outcomes, using patient data to optimize scheduling and care delivery. The ideal candidate has a Bachelor's Degree, at least three years in social services, CCM or CHW certifications preferred, and bilingual English/Spanish.

Qualifications

• Bachelor's Degree required.
• Three years' experience in social service setting providing targeted care management to multi-ethnic communities.
• CCM or CHW certification preferred.
• Bilingual English/Spanish strongly preferred, consistent with patient population served.

Responsibilities

• Supports BH Fusion Lab goals of increasing access, quality, and growth by helping providers reduce no-shows, increase completed visits, and improve metrics.
• Tracks and analyzes patient-level and site-level data on patient flow, scheduling, no-show, and referral outcomes; identifies breakdowns and implements actions.
• Oversees provider schedules across all IBH services, including templates, reminders, waitlists, and backfill, and supports rapid-cycle improvement activities.
• Coordinates care monitoring with families and maintains contact between families and service providers to monitor progress.
• Ensures continuity of care across IBH programs, primary care, psychiatry, recovery services, and transitions of care; performs document requests and uploads.
• Provides Spanish interpretation as needed.
