Responsibilities

• Coordinate scheduling for Behavioral Health services, including new patient appointments, follow-up appointments, and individual and group therapy appointments.
• Receive and process Behavioral Health referrals from HPC providers and outside referral sources, ensuring referrals are appropriately documented and routed.
• Coordinate with outside behavioral health agencies and providers regarding referrals, appointments, treatment needs, and continuity of care.
• Obtain and coordinate medical records and other required clinical documentation for new Behavioral Health patients referred from outside organizations or providers.
• Verify patient demographics, insurance coverage, financial responsibility, and applicable copayments prior to scheduled services.
• Communicate financial responsibility and payment expectations to patients in accordance with HPC policies and procedures.
• Ensure patients complete required steps and meet established Behavioral Health program requirements prior to appointments.
• Communicate with patients regarding appointments, referrals, required documentation, and other steps necessary to access Behavioral Health services.
• Maintain accurate and timely documentation of referrals, scheduling, patient contacts, outside records, and other care coordination activities in the electronic health record.
• Maintain current information regarding community Behavioral Health, substance use disorder, and social service resources available to HPC patients.
• Maintain positive working relationships with outside referral agencies and community partners.
• Assist with monitoring department workflows and identifying opportunities to improve patient access, scheduling, referral management, and continuity of care.
• Participate in Behavioral Health, clinic, community partner, and other meetings and trainings as appropriate.
• Maintain strict confidentiality of patient information and comply with HIPAA and all applicable HPC policies and procedures.
• Represent Health Partnership Clinic professionally when communicating with patients, providers, referral partners, and community organizations.
• Perform other duties as assigned.
• Regular attendance and punctuality are essential requirements of this position.
