Job summary

Southwest Behavioral & Health Services is seeking a Care Coordinator for the Mesa Outpatient Clinic to connect with members, learn their stories, and help improve quality of life. The role involves crisis stabilization, care coordination, and collaboration with the clinical team.

Responsibilities include developing service plans, coordinating care with treatment teams, and ensuring proper documentation. Requires a Bachelor’s degree in behavioral health and at least 1 year of related experience,

Qualifications

• Bachelor’s degree in behavioral health required or an equivalent combination of education and experience (5 years total).
• 1 year of full-time behavioral health experience required.
• Must have fingerprint clearance or be eligible to obtain within 8 weeks of hire.

Responsibilities

• Interventions may be individual, group-based, or community collaborative.
• Uncompromisingly coordinate care between resources inside and outside the agency.
• Develop service plans with consumers based on assessment and diagnostic process.
• Monitor mental status and update treatment plans as needed.
• Provide face-to-face crisis intervention and safety planning.
• Coordinate hospital discharges and arrange follow-up appointments.
• Maintain thorough documentation of contact with consumers and services received.
• Attend weekly clinical staffing and staff meetings.

Education

Bachelor’s degree in behavioral health or related field
