Job summary

UnitedHealth Group is seeking a patient-facing care coordinator to support coordinated, patient-centered care for at-risk individuals. You will liaise between primary care, behavioral health teams and families, track appointments, and document in the electronic medical record.

The role emphasizes HIPAA compliance, data analysis and cross-site collaboration in a fast-paced environment, with office-based duties and weekly onsite presence.

Qualifications

• High School Diploma or GED required.
• 1+ year experience in a social services or behavioral health setting.
• 1+ year experience with electronic medical records (EMR).
• Proficiency with Microsoft Office (Word and Excel).
• Bachelor's degree preferred in healthcare administration, psychology or related field.

Responsibilities

• Facilitates care processes for at-risk patients and coordinates an accessible plan of care.
• Serves as liaison between primary care team, behavioral health clinicians and patient/family.
• Assesses patient circumstances, health literacy, language and cultural beliefs affecting care; documents in EMR.
