Responsibilities

• Facilitates care process for at-risk patients and tracks adherence.
• Acts as liaison between primary care, behavioral health, patient, and family.
• Documents patient circumstances and health literacy in medical records.
• Tracks appointments, labs, referrals, and prescription compliance.
• Generates data reports and presentations for clinicians.
