Job summary

UnitedHealth Group's Optum is seeking a care coordinator to support coordinated, patient-centered care for at-risk individuals. You will facilitate care plans, monitor adherence, and serve as a liaison between primary care, behavioral health teams, and patients or their families.

This role is office-based, with some on-site work weekly and travel as needed. You will document in EMR, track appointments and tests, and present data to clinicians.

Qualifications

• High School Diploma or GED.required.
• 1+ years of experience in a social services or behavioral health setting.
• 1+ years of experience with electronic medical records (EMR).
• Intermediate level of proficiency with Microsoft Office (Word and Excel).
• Bachelor's degree in related field preferred.

Responsibilities

• Facilitates the process of care for at risk patients, coordinates an accessible plan of care and tracks adherence.
• Serves as a liaison between the primary care team, behavioral health clinicians and patient and/or patient's family.
• Documents in medical records and shares information with health care team.
• Tracks adherence to appointments, labs, studies, referrals, and prescription compliance.
• Analyses data, prepares reports and presentations for clinicians.
• Contacts patients via telephone, MyHealth and/or direct mailings; guides through care transitions.
• Ensures all necessary documentation is entered into the EMR.
• Obtains records from providers and schools for patient visits.
• Identifies community resources and distributes to patients.
• Documents processes and participates in care-improvement activities.
• Protects PHI and complies with HIPAA privacy rules.
