Qualifications

• High School Diploma or GED required.
• 1+ years of experience in a social services or behavioral health setting.
• 1+ years of experience with electronic medical records (EMR).
• Intermediate proficiency with Microsoft Office (Word and Excel).

Responsibilities

• Facilitates care processes for at-risk patients and tracks adherence.
• Serves as liaison between primary care, behavioral health clinicians, and patients.
• Documents patient circumstances and health literacy in the medical record.
• Tracks appointments, labs, referrals, and prescription adherence.
• Extracts data and creates presentations to inform clinicians.
