Job summary

Health Link Home Health & Hospice is seeking an experienced home health sales leader to grow our Bay Area division. You will lead and coach a business development team, shape growth strategies with leadership, and manage relationships with hospitals, physicians, and referral sources.

The role requires home health experience, a track record of meeting targets, and strong Medicare knowledge. A hunter mindset and data-driven approach will help drive census growth and market development.

Qualifications

• Minimum 2 years of home health experience.
• 2+ years managing or leading a sales or business development team.
• Strong working knowledge of Medicare, Medicare Advantage, commercial insurance, and managed care in home health.
• Familiarity with PDGM and home health eligibility and referral requirements.
• Proven track record of meeting or exceeding referral and growth targets.
• Established relationships with Bay Area referral sources (preferred).
• Experience with CRM, reporting tools, and Google Workspace.
• Excellent communication, presentation, and organizational skills.
• Valid driver's license, auto insurance, and reliable transportation.

Responsibilities

• Lead the team: coach and develop business development representatives; joint sales calls; account management; field problem resolution.
• Drive growth: partner on strategy; identify opportunities in existing/new referral accounts.
• Build relationships: hospitals, physicians, case managers, SNFs, assisted living, and other referral sources.
• Own the data: monitor trends, census, and metrics; prepare reports for leadership.
• Strengthen referral conversion: collaborate with Intake, Clinical Operations, and Leadership; represent Health Link in events.
• Hit goals: support census growth, referral volume, and market development.
