Job summary

Comprehensive Rehab Consultants (CRC) seeks a PM&R/Physiatry clinician to provide specialized rehabilitative services at a designated SNF or post-acute rehab center in Modesto, CA. You will work closely with the post-acute therapy team to optimize outcomes and promote patient independence.

Qualifications

• Post-acute space experience preferred.
• Ability to function autonomously in SNF/post-acute settings.
• Strong interpersonal and collaboration skills with interdisciplinary teams.

Responsibilities

• Provide PM&R/Physiatry services at a designated SNF or post-acute rehab center.
• Collaborate with PCPs and therapy teams to manage common SNF conditions.
• Lead and represent CRC in the facility through clinical leadership.
