Comprehensive Rehab Consultants in Chico, CA is seeking a Nurse Practitioner/APP or Physician Assistant to provide PM&R/Physiatry services at a designated SNF. You will lead with clinical excellence, collaborate with the care team, and support post-acute rehabilitation goals.

The role offers a 40-hour workweek with daytime hours around 9 AM to 4 PM, generous benefits, education reimbursement, and a path to clinical leadership within CRC.

Qualifications

• Must be NP/PA with California license.
• Post-acute/SNF experience preferred.
• Autonomy, compassionate, and team-oriented healthcare provider.
• Eligible to work in the U.S.
• 40 hours per week.

Responsibilities

• Provide PM&R/Physiatry services as a rehab consultant.
• Represent CRC in your facility with clinical leadership and collaboration with the care team.
• Collaborate with PCP teams on SNF conditions such as CVA, progressive neurological disorders, and postoperative surgeries.
• Coordinate with PT/OT/SLP teams to improve patient function and quality of life.
• Attend multidisciplinary meetings and bridge therapy with internal medicine.
• Engage in culture building and ongoing education.
