Job summary

Comprehensive Rehab Consultants (CRC) is seeking a Nurse Practitioner/ APRN or PA to provide PM&R/Physiatry services at a designated SNF or post-acute rehab center in San Diego, CA.

The role emphasizes clinical leadership, autonomous practice, and collaboration with an interdisciplinary team, including PT, OT, and SLP. Expect a flexible schedule around 9 AM–4 PM with competitive salary and bonus opportunities.

Qualifications

• Experience in post-acute space is preferred.
• Ability to work autonomously in SNF/post-acute settings.
• Strong interpersonal skills with facility partners and patients.

Responsibilities

• Provide PM&R/Physiatry services as a rehab consultant.
• Lead clinically within the facility and collaborate with the care team.
• Coordinate with PCPs to manage SNF-relevant conditions.
• Work with PT/OT/SLP to improve patient function and quality of life.
• Attend multidisciplinary meetings and bridge therapy with internal medicine.
• Promote culture and ongoing education within the team.
