Job summary

21st Century Home Health Services in the Fremont area is seeking a Full Time Physical Therapist Case Manager, Level 2 to evaluate and treat patients in their homes. You will supervise two PTAs and coordinate with physicians to ensure high-quality home care.

The role emphasizes autonomy, flexible scheduling, and a comprehensive benefits package including 401k with match, generous PTO, and insurance; candidates must hold a CA PT license and BLS/CPR.

Qualifications

• Current unrestricted CA Physical Therapist License.
• 1+ year experience supervising PTAs in a HH environment.
• Current BLS/CPR Certification.
• Valid CA Driver's License.
• Knowledge of local community resources.

Responsibilities

• Perform an evaluation in the home and develop a care plan and work with PTAs to follow the care plan.
• Help patients with mobility, movement, and help in regaining their independence.
• Create detailed treatment and care plans for each patient.
• Communicate and coordinate with patients’ physicians to ensure a quality level of care.
• Evaluate and report patients’ progress to MD
