Job summary

21st Century Home Health Services is seeking a Full-Time Physical Therapist Case Manager, Level 2, for the Oakland area. You will evaluate patients in their homes, develop care plans with PTAs, and coordinate with physicians to ensure high-quality care.

The role requires an active CA PT license, BLS/CPR, and at least 1 year supervising PTAs in HH. A driver’s license and knowledge of local resources are essential for success.

Qualifications

• Active, unrestricted California Physical Therapist License.
• Experience supervising PTAs in home health (1+ year a plus).
• Current BLS/CPR certification.
• Valid California driver's license.
• Knowledge of local community resources and regulatory guidelines.

Responsibilities

• Evaluate patients in their homes and create care plans with PTAs.
• Address mobility and independence through therapy.
• Coordinate with physicians and other clinicians.
• Supervise PTAs to ensure performance standards.
• Document progress and adjust plans as needed.
