Job summary

21st Century Home Health Services is seeking a Full-Time Physical Therapist Case Manager, Level 2 for the Folsom area. You will evaluate patients in their homes, develop care plans, and supervise 2 PTAs to help patients regain function and independence while ensuring high‑quality care.

Requirements include CA PT license, 1+ year PTA supervision, BLS/CPR, valid CA driver's license, and knowledge of local resources. The role offers generous benefits and a performance bonus.

Qualifications

• Current unrestricted CA Physical Therapist License.
• 1+ year experience supervising PTAs in a HH environment is a plus.
• Current BLS/CPR Certification.
• Valid CA Driver's License.
• Knowledge of local community resources.
• Knowledge of culture and health care in home settings.
• Experience with high acuity patients and interdisciplinary teams is desirable.

Responsibilities

• Evaluate in-home and develop a care plan with PTAs to follow.
• Help patients regain independence and mobility.
• Create detailed treatment and care plans for each patient.
• Coordinate with physicians to ensure quality care.
• Evaluate and report patient progress to MD.
