Job summary

21st Century Home Health Services is seeking a Physical Therapist Case Manager, Level 2, to oversee home-based therapy services in the Windsor, CA area. You will evaluate patients, plan treatments, and supervise PTAs to help clients regain function at home.

The role requires a CA license, CPR/BLS, and strong collaboration with interdisciplinary teams. Competitive benefits and incentive bonuses are offered.

Qualifications

• CA Physical Therapist license required.
• 1+ year supervising PTAs in home health preferred.
• Current BLS/CPR certification.
• Valid CA Driver's License.
• Knowledge of local community resources required.
• Experience with high acuity patients is a plus.
• Experience with an interdisciplinary team is highly desired.

Responsibilities

• Perform a home evaluation and develop a care plan; work with PTAs to follow the plan.
• Help patients with mobility and regain independence.
• Create detailed treatment and care plans for each patient.
• Coordinate with patients’ physicians to ensure quality care.
• Evaluate and report patients’ progress to MD.
