Job summary

21st Century Home Health Services (21HHS) is seeking a Full Time Physical Therapist Case Manager, Level 2 in the Alameda, CA area. You will evaluate patients in their homes, create care plans, supervise PTAs, and coordinate with physicians to ensure high-quality care.

This role emphasizes autonomy, flexible scheduling, and territory-based assignments, with competitive compensation, comprehensive benefits, and an annual bonus program. A valid CA license and BLS/CPR are required.

Qualifications

• Current unrestricted CA Physical Therapist License is required.
• 1+ year experience supervising PTAs in a HH environment is a plus.
• Current BLS/CPR Certification is required.
• Valid CA Driver's License is required.
• Knowledge of local community resources is required.
• Experience with high acuity patients is a plus.
• Experience working with an interdisciplinary team is highly desired.

Responsibilities

• Perform an evaluation in the home and develop a care plan with PTAs.
• Help patients with mobility and independence.
• Create detailed treatment and care plans for each patient.
• Communicate with patients’ physicians to ensure quality care.
• Evaluate and report patients’ progress to the MD.
