Job summary

21st Century Home Health Services in Concord, CA seeks a Physical Therapist Case Manager, Level 2 to deliver skilled in-home PT services. You will evaluate patients, address needs, and supervise PTAs to ensure care plans are followed.

You will collaborate with physicians, create treatment plans, and help patients regain independence while maintaining high-quality home health care. CA PT license and BLS/CPR required.

Qualifications

• Current unrestricted CA Physical Therapist License.
• 1+ year experience supervising PTAs in a home health setting is a plus.
• Current BLS/CPR Certification.
• Valid CA Driver's License.
• Knowledge of local community resources.
• Knowledge of home health regulatory guidelines preferred.
• Experience with high acuity patients a plus.
• Experience working with an interdisciplinary team is highly desired.
• Case Management fundamentals are highly desired.

Responsibilities

• Perform an evaluation in the home and develop a care plan and work with PTAs to follow the care plan.
• Help patients with mobility, movement, and regain independence.
• Create detailed treatment and care plans for each patient.
• Coordinate with physicians to ensure a quality level of care.
• Evaluate and report patients’ progress to MD.
