21st Century Home Health Services in Alameda, CA is seeking a Full-Time Physical Therapist Case Manager, Level 2. You will evaluate and treat patients in their homes, supervise PTAs, and coordinate with physicians to maintain high-quality care.

Qualifications

• Current unrestricted CA Physical Therapist License.
• 1+ year experience supervising PTAs in a HH environment.
• Current BLS/CPR Certification.
• Valid CA Driver's Licence.
• Knowledge of local community resources.
• Experience with high-acuity patients is a plus.
• Experience working with an interdisciplinary team is highly desired.

Responsibilities

• Evaluate and address home-based patient needs to restore function.
• Supervise PTAs and ensure care plan adherence.
• Develop and implement individual treatment plans.
• Coordinate with physicians to ensure quality care.
• Document progress and outcomes.
• Navigate electronic charting and company systems.
