Job summary

21st Century Home Health Services in Martinez, CA seeks a Full Time Physical Therapist Case Manager, Level 2 to evaluate and treat patients in home settings, supervising PTAs and coordinating with physicians.

You will work in a field-based role with flexible scheduling, mileage reimbursement, competitive pay, and comprehensive benefits including 401k, medical, dental, life insurance, EAP and FSAs.

Qualifications

• CA Physical Therapist license required.
• 1+ year supervising PTAs in HH environment preferred.
• Current BLS/CPR certification.
• Valid CA driver's license.
• Knowledge of local community resources.
• Experience with high-acuity patients and interdisciplinary teams preferred.
• Self-directed and empathetic with strong communication.

Responsibilities

• Perform home evaluations and develop care plans with PTAs.
• Help patients regain mobility and independence.
• Create detailed treatment plans for each patient.
• Coordinate with physicians to ensure quality care.
• Report patient progress to the MD.
