Qualifications

Responsibilities

• Evaluate and address patient needs in the home to restore function and independence.
• Supervise PTAs to ensure quality care and adherence to plan of care.
• Develop detailed treatment and care plans.
• Collaborate with physicians to coordinate high-quality care.
• Report patient progress to the treating MD.
