GEO Reentry Services LLC is seeking an Assistant Case Manager to provide front-line case management support at our Reentry Service Center. You will check in participants, assist Case Managers and staff, and help coordinate services, while applying evidence-based practices to support successful outcomes.

The role emphasizes data entry, reporting, and collaboration with a multidisciplinary team to monitor progress and ensure high-quality service delivery.

Responsibilities

• Checks-ins all participants entering the facility and documents results.
• Performs breath analysis and records results in GEOTrack/Company database.
• Completes Starting Point process with participants and enters data into the system.
• Provides general office administration and customer service to participants and the public.
• Assists RSC staff with case coordination and reporting.
• Manages caseloads and develops individual treatment plans.
• Facilitates CBT programs and tracks attendance and progress.
• Documents progress and maintains records in GEOTrack/Company database.
