St. Joseph’s Health in Paterson, NJ seeks an Ambulatory Patient Navigator I to support coordinated, patient-centered care across the ambulatory Primary Care network. You will engage in patient outreach, navigation, and care coordination, helping with transitional care and referral activities.

The role collaborates with providers, nursing, operations, and interdisciplinary teams to advance value-based care within a PCMH framework and improve patient outcomes and satisfaction.

Qualifications

• High School Diploma or equivalent required.
• Associate’s degree in Healthcare Administration, Public Health, or related field preferred.
• Completion of Medical Assistant Program or CMA Certification OR EMT certification.
• Minimum 1 year of healthcare experience; ambulatory setting preferred.
• Experience with electronic medical records preferred.
• Excellent communication, interpersonal, organizational and customer service skills.
• Ability to manage multiple priorities in a fast-paced ambulatory environment.
• Ability to work collaboratively in interdisciplinary teams.
• Knowledge of ambulatory workflows and value-based care initiatives; bilingual abilities preferred.

Responsibilities

• Support coordinated, patient-centered care across the ambulatory Primary Care network.
• Assist with patient outreach, navigation, care coordination and transitional care support.
• Support APCM, TCM, preventive care initiatives, chronic disease management, and referrals.
• Contribute to care gap closure workflows to improve outcomes and patient experience.
• Collaborate with providers, nursing, operations, and interdisciplinary teams.
