The role works with the Nurse Navigator and health care professionals to assess patient needs, navigate the system, and optimize access to care while demonstrating compassion, professionalism, and strong communication skills.

Qualifications

• Associates Degree preferred; Patient Navigator Certification preferred or obtained within 1 year.
• Basic understanding of medical terminology and health care financing.
• Proficiency with a personal computer.
• Fluency in English.

Responsibilities

• Provide individualized service and navigate patients through the health system.
• Collaborate with the Nurse Navigator and other health care professionals to evaluate and address patient needs.
• Ensure timely access to services and optimize health and wellness for patients.
