Responsibilities

• Proactively identifies patients for health maintenance and chronic care management.
• Leads patient outreach for preventive services and follow-ups.
• Supports primary care pre-visit planning and quality reporting.
• Facilitates access to primary care and preventive services for patients and families.
• Reports to Population Health Performance Manager and collaborates with care teams.
