Job Description

The Ambulatory Patient Navigator II serves as an advanced care coordination resource supporting high-risk, chronic disease, transitional care, and value-based care initiatives across the ambulatory Primary Care network. This role independently manages more complex patient navigation and care coordination activities while supporting quality improvement, Patient-Centered Medical Home (PCMH) initiatives, APCM, TCM, and population health strategies designed to improve patient outcomes, access, and continuity of care.

Qualifications

• High School Diploma or equivalent required.
• Associate’s degree in Healthcare Administration, Public Health, or related healthcare field required.
• Bachelor’s degree in Healthcare Administration, Public Health, or related healthcare field preferred.

Experience

• Minimum 3 to 5 years of experience working in healthcare, ambulatory outpatient practices, patient access, care coordination, EMS or related field preferred.
• Experience working within an electronic medical record preferred.

Active Emergency Medical Technician (EMT) certification.

Knowledge, Skills, And Abilities

• Strong communication and interpersonal skills.
• Strong organizational and customer service skills.
• Ability to manage multiple priorities in a fast-paced ambulatory environment.
• Ability to work collaboratively within interdisciplinary teams.
• Knowledge of ambulatory workflows, patient navigation, and value-based care initiatives preferred.
• Bilingual abilities preferred based on practice needs.
