St. Joseph’s Health seeks an experienced nurse navigator to coordinate resources for patients and families, with a focus on seamless care transitions and education. The role involves collaboration with multidisciplinary teams to ensure timely access to services and adherence to care plans.

Required: Bachelor’s in Nursing, NJ RN licensure, 3–5 years of related experience, and BCLS and ONS certifications within 1 year of hire. This position is based in New Jersey.

Qualifications

• Bachelor's degree in Nursing and RN licensure required.
• 3–5 years of related work experience.
• BCLS certification and Chemotherapy/Biotherapy Provider (ONS) within 1 year of hire.

Responsibilities

• Serves as the primary contact for physicians, patients and families to coordinate access to clinical, supportive, and financial services.
• Coordinates multidisciplinary team to ensure continuity of care and timely services.
• Facilitates transitions of care with appropriate referrals and follow-up appointments.
• Delivers patient education to promote adherence to the plan of care and outcomes.

Education

Basic Cardiac Life Support (BCLS)
