Job summary

Libertana is seeking an ALF Case Manager in California to coordinate care for ALWP clients, verify Medi-Cal eligibility monthly, and arrange visits. You will speak with clients, document details, and ensure timely updates in the care tracking system. Driving across covered counties may be required; collaboration with staff is essential.

You should have a relevant bachelor’s degree, CPR certification, and familiarity with HIPAA; strong organizational and communication skills are needed to succeed.

Qualifications

• Bachelor’s Degree in social work, psychology, counseling, rehabilitation, gerontology, or sociology.
• Possesses current CPR certification.
• Knowledge of Confidentiality, HIPAA and healthcare laws and regulations.
• Familiarity with Title 22 preferred.
• Must be a licensed driver with an automobile in good working order.

Responsibilities

• Check Medi-Cal eligibility monthly.
• Schedule monthly visits with clients.
• Update client information on company tracking system and ensure notes uploaded by next business day.
• Notify ALF Coordinator when a client leaves or returns to a facility.
• Attend state mandated trainings and department meetings.
