Job summary

Libertana in California seeks an ALF Case Manager to coordinate care for ALWP clients within the Care Coordination Agency. You will identify, organize, and monitor client needs, ensuring continued Medi-Cal eligibility and timely documentation in Devero.

Qualifications

• Bachelor’s degree in a related field plus one year of related work experience.
• Current CPR certification.

Responsibilities

• Check Medi-Cal eligibility monthly.
• Schedule monthly visits with clients.
• Speak with clients to assess needs (e.g., DME, appointments).
• Update client information in the company tracking system and ensure notes are uploaded.
• Ensure notes are completed by the end of the next business day.
• Notify ALF Coordinator when a client leaves or returns to a facility.
• Copy ISP during re-assessment and obtain client signatures within 7 business days.
• Maintain confidentiality and comply with HIPAA and healthcare laws.
