Job summary

Libertana is seeking an ALF Case Manager to coordinate care for ALWP clients, conducting eligibility checks, arranging visits, and ensuring accurate documentation. The role requires up-to-date credentials, strong organizational skills, and ability to drive across multiple counties.

Qualifications

• Bachelor’s degree in social work, psychology, counseling, rehabilitation, gerontology, or sociology plus one year of related work experience.
• CPR certification is required.
• Knowledge of California Assisted Living Waiver is preferred.

Responsibilities

• Check Medi-Cal eligibility for each client monthly.
• Schedule monthly visits with clients.
• Update client information and notes in Devero; ensure timely uploads.
• Coordinate SIRs and ensure staff follow up for SIR copies.
• Communicate Medi-Cal issues to clients and facility staff; guide resolution.
• Maintain confidentiality and HIPAA adherence; follow policies and procedures.
• Drive to service areas as needed; track client movements and notify if client leaves/returns.
• Assist with Re-Assessment ISP processes and obtain client signature within 7 business days from RN notification.
