Job summary

Paycom is seeking an ALF Case Manager in Cypress, CA to coordinate care for ALWP clients under the Care Coordination Agency. Responsibilities include monthly Medi-Cal eligibility checks, scheduling visits, and ensuring timely documentation in Devero.

Strong knowledge of HIPAA, excellent judgment, and solid organizational skills are required. The role involves coordinating with staff and facilities, driving within specified counties, and maintaining up-to-date credentials.

Qualifications

• Bachelor’s Degree in social work, psychology, counseling, rehabilitation, gerontology, or sociology plus 1 year related experience.
• CPR certification.
• Knowledge of confidentiality, HIPAA and healthcare laws.
• Experience in education, training, QA and management preferred.
• Excellent judgment and clinical documentation skills.
• Good organizational skills.
• Self-directed with minimal supervision.
• Proficient in computers and electronic information systems.
• Licensed driver with insured vehicle in good condition.

Responsibilities

• Check Medi-Cal eligibility for each client monthly.
• Schedule monthly visits with clients.
• Assess medication compliance and needs; coordinate DME or appointments.
• Update client info in Devero; ensure notes uploaded by next business day.
• Notify ALF Coordinator when a client leaves and returns to facility.
• For Re-Assessments, obtain ISP, have client sign, and distribute copies to facility.
