Job summary

Libertana seeks an ALF Case Manager in Orange County to coordinate care for ALWP clients under CCA. You will verify Medi‑Cal eligibility, arrange monthly visits, assess medication adherence, and monitor SIR compliance while maintaining up‑to‑date records in Devero.

You will collaborate with facility staff, ensure proper documentation, and support re‑assessments with ISP handling. A valid CA driver’s license and CPR certification are required.

Qualifications

• Bachelor’s degree in social work or related field.
• Current CPR certification.
• Knowledge of California Assisted Living Waiver Care Coordination Agency preferred.
• Experience in education, quality assurance or management preferred.
• Judgment and clinical documentation skills.
• Good organizational skills and self-direction.
• Proficient in computers and electronic information systems.
• Ability to communicate well with office and field staff.
• Familiarity with Title 22 preferred.
• Must be a licensed driver with a valid insured vehicle.

Responsibilities

• Check Medi‑Cal eligibility monthly and schedule visits.
• Discuss client needs, medications, and care requirements.
• Coordinate with staff to obtain SIRs and follow up on gaps.
• Inform clients and facilities about Medi‑Cal issues and resolutions.
• Update client information in the company tracking system and Devero.
• Document notes promptly and confirm med‑compliance discussions.
• Track client status when leaving/returning to facilities and notify ALF Coordinator.
• Assist with re-assessments; ensure ISP copies are signed and filed.
