Job summary

Libertana is seeking an ALF Case Manager to coordinate care for ALWP clients under the Care Coordination Agency. This role involves verifying Medi-Cal eligibility, scheduling client visits, and coordinating with facilities to ensure service continuity.

You will document thoroughly in Devero, update client records promptly, and communicate with staff about medication adherence and required authorizations. A valid CA driver’s license and CPR certification are required.

Qualifications

• Bachelor's Degree in social work, psychology, counseling, rehabilitation, gerontology, or sociology is required.
• CPR certification is required.
• Familiarity with Title 22 is preferred.
• Experience in education, training, quality assurance or management is preferred.
• Excellent judgment with strong clinical, analytical and documentation skills.
• Good organizational skills and the ability to work with minimal supervision.

Responsibilities

• Check Medi-Cal eligibility for each client monthly.
• Schedule monthly visits with clients.
• Engage with clients about needs (DME, appointments, medications).
• Monitor med adherence and document in the tracking system (Devero).
• Record notes and ensure upload by the end of the next business day.
• Notify ALF Coordinator when client leaves and returns to facility.
• Copy ISP after Re-Assessment, have client sign, give to facility, and upload to Devero.
• Maintain confidentiality and comply with HIPAA and healthcare laws.
• Attend mandated trainings and maintain credentials.
