Job summary

Libertana is seeking an Office Coordinator for the Assisted Living Waiver Program Care Coordination Agency (ALWP CCA) to process beneficiary applications in accordance with DHCS ALWP Policy and Libertana policies.

The role requires coordinating data, completing State Waiver Program applications, maintaining compliance with HIPAA and regulatory standards, and fostering relationships with RCFE and SNFs while supporting staff and supervisors.

Qualifications

• Fluent in English with strong written and verbal skills.
• Excellent judgment and organizational abilities.
• Proficient with computers, EMR, and MS Office.
• Licensed driver with reliable vehicle as per policy.

Responsibilities

• Assist Medi-Cal beneficiaries in obtaining access to waiver services and coordinating data for ALWP applications.
• Complete and ensure accuracy of all documents for State Waiver Program Applications.
• Maintain compliance with HIPAA and regulatory standards; support staff and supervisors.
• Build and maintain relationships with RCFE/SNFs and other referral sources.
