Job summary

Paycom - ATS is seeking an ALF Case Manager to coordinate care for ALWP clients under the Care Coordination Agency in California. You will assess needs, verify Medi-Cal eligibility, schedule monthly visits, and monitor compliance and ISP processes with facility staff.

Responsibilities include maintaining accurate visit logs in Devero, ensuring timely notes, tracking SIRs, and coordinating reassessments with the RN.

Qualifications

• Bachelor-level degree in a related field and CPR certification.
• Must be able to maintain HIPAA/confidentiality standards and high ethical conduct.

Responsibilities

• Check Medi-Cal eligibility for each client monthly.
• Schedule monthly visits with clients and document findings.
• Coordinate with staff to obtain SIRs and ensure copies are available.
• Update client information in Devero and upload notes by the next business day.
• Track client movements (out of facility) and notify ALF Coordinator when needed.
• Assist with ISP reassessment coordination and ensure copies are signed and distributed.
