Job summary

Libertana is seeking a fully remote Care Coordinator to support the California Assisted Living Waiver Program. You will assist Medi‑Cal beneficiaries, coordinate ALWP applications, and maintain approved documentation across RCFE and SNF networks.

The role requires strong communication, organizational and analytical skills, with a focus on compliance and data accuracy. A driver’s license and healthcare administration background are preferred.

Qualifications

• Knowledge of the State of California Assisted Living Waiver Program Care Coordination Agency Provider Handbook, RCFE Provider Handbook and Title 22 Division 6 for RCFE.
• Excellent judgment and organizational skills.
• Able to communicate effectively in English (written and verbal).
• Proficient with computers, electronic medical records, and Microsoft Office.

Responsibilities

• Assist Medi-Cal beneficiaries in accessing waiver services and data collection for ALWP applications.
• Ensure proper completion of all State Waiver Program documents.
• Review ALWP documents including Initial Assessments, ISPs, and Medi‑Cal eligibility.
• Coordinate with RCFE managers, SNFs and referral sources to resolve issues.
• Maintain patient charts and ensure HIPAA compliance.
• Develop and maintain relationships with RCFE, SNFs and vendors.
• Ensure compliance with Federal, State, Local, HIPAA, and Joint Commission standards.
