Job summary

Gentiva is seeking an Afterhours RN Care Manager to provide high-quality hospice care during nights, weekends, and on-call hours. You will perform after-hours assessments, routine visits, admissions, and live discharges while coordinating with the IDG to ensure continuity of care.

The role requires an active RN license, graduation from an accredited nursing program, and at least one year of RN experience in hospice or related settings. Travel and on-call responsibilities are part of the schedule.

Qualifications

• RN license in the state of employment.
• Graduate of an accredited nursing program.
• Minimum one year of RN experience in acute care, oncology, long-term care, hospice, or home health.

Responsibilities

• Provide direct in-person patient care during on-call, weekend, and after-hours shifts.
• Assess and coordinate patient and family needs.
• Maintain primary clinical oversight for assigned patients.
• Develop and revise the Hospice Plan of Care with physicians and IDG members.
• Educate patients and families on disease progression and symptom management.
