Job summary

Gentiva is seeking an Afterhours RN Care Manager to provide high-quality hospice care outside standard business hours, ensuring seamless coverage and strong clinical management for patients and families during nights, weekends, and on-call hours.

The role involves direct after-hours care, coordination with the IDG, and ongoing education of patients and families, while maintaining thorough documentation and compliance across shifts.

Qualifications

• Current RN license in the state of employment.
• Graduate of an accredited nursing program.
• Minimum one year of RN experience in acute care, oncology, long-term care, hospice, or home health.

Responsibilities

• Provide direct in-person patient care during after-hours shifts, including assessments and admissions.
• Assess and coordinate patient and family needs after hours.
• Develop and revise the Hospice Plan of Care with physicians and IDG members.
• Educate patients and families on disease progression, symptom management and hospice philosophy.
• Maintain accurate clinical documentation and care plans.
