Job summary

Gentiva Hospice is seeking an Afterhours RN Care Manager to deliver high-quality hospice care outside standard business hours, ensuring seamless coverage, continuity of care, and strong clinical management for patients and families during nights, weekends, and on-call hours.

The role requires after-hours assessments, care coordination with the IDG, education for families, and meticulous documentation, while integrating with physicians and staff to uphold hospice standards.

Qualifications

• Current RN license in the state of employment.
• Graduate of an accredited nursing program.
• Minimum one year of RN experience in acute care, oncology, long-term care, hospice, or home health.

Responsibilities

• Provide direct in-person patient care during after-hours shifts, including assessments, admissions, routine visits, and death visits.
• Coordinate with physicians and IDG to develop and update plan of care.
• Maintain accurate clinical documentation and ensure timely charting.
