Job summary

Gentiva seeks an Afterhours RN Care Manager to deliver high-quality hospice care during nights, weekends, and on-call hours. You will assess patients, coordinate with the Interdisciplinary Group, and maintain primary clinical oversight on assigned cases.

The role requires an active RN license, CPR certification, and strong communication with families and clinicians to ensure compassionate, compliant care. Training and support are provided.

Qualifications

• Thorough patient assessments and development of individualized care plans.
• RN license in the state of employment and CPR certification.
• Experience in hospice, home health, or related settings beneficial.

Responsibilities

• Provide direct in-person patient care during on-call, weekend, and after-hours shifts.
• Conduct assessments of physical, emotional, spiritual, and psychosocial needs.
• Support IDG coordination and update plans of care with physicians and team.
