Job summary

Gentiva Hospice seeks an Afterhours RN Care Manager to deliver high-quality, compassionate hospice care outside standard hours. You will lead direct patient assessments, coordinate with the IDG, and ensure seamless clinical oversight for nights, weekends, and on-call shifts.

The role emphasizes comprehensive symptom management, charting, and education for patients and families, while maintaining regulatory compliance and quality outcomes. A Nurse license and hospice experience are required.

Qualifications

• Current RN license in the state of employment.
• Graduate of an accredited nursing program.
• Minimum one year of RN experience in acute care, oncology, long-term care, hospice, or home health.

Responsibilities

• Provide direct in-person hospice care during after-hours shifts, including assessments and visits.
• Coordinate care with physicians, families, and the IDG to updates plans of care.
• Maintain accurate, timely clinical documentation and chart reviews.
• Support scheduling, triage, and visit reassignment per patient plan of care.
