Job summary

Gentiva is seeking an Afterhours RN Care Manager to provide high-quality hospice care outside standard hours. You will assess patients, coordinate with the IDG, and maintain primary clinical oversight for after-hours cases.

The role requires an active RN license, at least one year of RN experience in relevant settings, and strong knowledge of hospice principles. Comprehensive benefits and a competitive salary range are offered.

Qualifications

• RN license in the state of employment
• Graduate of an accredited nursing program
• Minimum one year of RN experience in acute care, oncology, long-term care, hospice, or home health

Responsibilities

• Provide direct in-person hospice care during on-call, weekend, and after-hours shifts
• Assess physical, emotional, psychosocial, and spiritual needs
• Educate patients and families on disease progression and symptom management
• Maintain accurate clinical documentation and care plans
• Coordinate with physicians and IDG for plan of care updates
