Job summary

Gentiva Hospice is seeking an Afterhours RN Care Manager to provide high-quality hospice care outside standard hours. You will perform after-hours assessments, admissions, routine visits, and live discharges while maintaining clinical oversight for assigned patients.

You will collaborate with physicians and the IDG, educate families, and ensure timely documentation. The role emphasizes leadership, compassion, and compliance in a fast-paced home-care setting.

Qualifications

• Current RN license in the state of employment
• Graduate of an accredited nursing program
• Minimum one year of RN experience in acute care, oncology, long-term care, hospice, or home health

Responsibilities

• Provide direct in-person care during after-hours, on-call, weekends and holidays
• Assess physical, emotional, psychosocial, and spiritual needs
• Coordinate care with physicians and IDG; update plans of care
• Educate patients and families on disease progression and symptom management
• Maintain accurate clinical documentation and chart reviews
• Lead field staff and participate in meetings, audits, and trainings
