Job summary

Gentiva Hospice seeks an Afterhours RN Care Manager to provide high-quality hospice care outside standard hours. This role ensures seamless coverage, continuity of care, and strong clinical oversight for patients and families during nights, weekends, and on-call hours.

The nurse will perform assessments, manage plans of care, educate families, and collaborate with the IDG to coordinate care after hours and maintain compliance and documentation.

Qualifications

• Current RN license in the state of employment.
• Graduate of an accredited nursing program.
• Minimum one year of RN experience in acute care, oncology, long-term care, hospice, or home health.

Responsibilities

• Provide direct in-person patient care during after-hours shifts, including assessments, admissions, routine visits, death visits, and live discharges.
• Assess physical, emotional, psychosocial, and spiritual needs of patients and families.
• Support the IDG process through chart reviews, care plan updates, and interdisciplinary communication.
• Develop and revise the Hospice Plan of Care with physicians and IDG members.
• Educate patients and families on disease progression, symptom management, medications, and hospice philosophy.
• Maintain accurate clinical documentation including assessments and care plans.
